Skilled Nursing
Professional nursing care and services requiring the skills of a registered or licensed practical nurse (under the supervision of a registered nurse).
Common examples include:
• Comprehensive health assessments
• Disease & medication management
• Wound, catheter, and ostomy care
• Monitoring after hospitalization
• Chronic condition management (CHF, COPD, Diabetes, etc.)
• Patient and caregiver education
Home Health Aide
• Bathing, grooming, dressing assistance
• Safe mobility & transfer support
• Toileting and personal care assistance
• Light meal preparation
• Basic home safety monitoring
Services Not Currently Offered
• Physical Therapy (PT)
• Occupational Therapy (OT)
• Speech-Language Pathology (SLP)
• Age 0 through 1 years old
